We'll start off emailing you our "Weight Loss" form for you to fill out. This form is what we'll use to collect all the information we need to get you on your weight loss track. The information you give us will help us understand what is holding you back from losing weight, and we'll do our best to kick start your weight loss program, or shift your current program into high gear.

- What are the top 3 benefits from losing the weight you want to lose?
- Do you have any additional benefits you'd like to list?
- How long has it been since you've weighed the amount you want to weigh?

- What is your mother's relationship with her weight?
- What is your father's relationship with his weight?
- How have you tried losing weight in the past?
- What are the different diets you have tried?
- Are you on a diet currently?
- Why do you think you are currently over the weight you want to be?
- Do you live with a significant other?
- If you have a significant other, how do they feel about your current weight?
- If you have a significant other, how do they feel about your desire to lose weight?
We'll record 3 different sessions just for you, a 10 minute session, a 20 minute session, and a 45 minute session, so that you can use whichever session you have time for.
